Make a complaint against the health service

The Health Service Ombudsman is an independent body that investigates complaints against the National Health Service (NHS) and local health authorities.

How to use this service

To make a complaint you can complete a complaint form from the Health Service Ombudsman website, which must be printed off for you to sign and posted back.
